The Licensing Application Process:

Each type of child care facility has its own licensing rules, specific procedures, and what the fee would be for becoming licensed. You may download, view, and print the Order Form, by using the free Adobe Acrobat Reader software, to order the application forms and rules for various types of child care facilities.

 

CHILD CARE CENTER LICENSE APPLICATION

Individuals interested in applying for a child care license to provide care in a child care center may download, view, and print the Child Care Center License Application and Affidavit (English version) or Affidavit (Spanish version) by using the free Adobe Acrobat Reader software.

 

FAMILY CARE HOME LICENSE APPLICATION  

Individuals interested in applying for a child care license to provide care in their homes may download, view, and print the Family Care Home License Application form and Affidavit (English version) or Affidavit (Spanish version) by using the free Adobe Acrobat Reader software.

 

A completed application package and Licensing Fee must be submitted to the Division of Child Care at least 90 days prior to the proposed opening date of the facility. After approval of the application package, a licensing specialist schedules an inspection of the facility, and determines whether or not a license should be issued.

 

Applicants, owners, employees, and residents of licensed child care facilities must also submit fingerprints to the Colorado Bureau of Investigation for a Criminal Background Check, and their names must be checked against the Central Registry of Child Protection.
